Men may change to:
- Mr (no proof required)
- Dr. - GMC (or GDC for Dentists) registration number will be required. Phd's and other doctors will need to submit other proof.
- Rev. - Proof will be required
- Prof. - Proof will be required
- Any other title you can provide valid proof of entitlement to. How to submit proof.
Women may change to:
- Miss (no proof required)
- Mrs (no proof required)
- Ms (no proof required)
